About Pacific Crest Homecare Solutions

Pacific Crest Homecare Solutions, LLC is part of a nationwide network, consisting of more than 300 in- home care agencies. Pacific Crest Homecare Solutions, LLC was founded by Dr. Samreen Fazalbhoy in 2017, to serve the needs of the senior and the disabled population.

Dr. Fazalbhoy has applied her education as a Doctor of Pharmacy, and her several years of experience of working as a Community Pharmacist, as well as a Research Pharmacist, to design a company that understands the needs of our clients and to ensure quality care.

Dr. Fazalbhoy leads a team of Certified Senior Care Managers and California state registered caregivers, who are highly skilled, experienced and passionate about senior care.
